Remote monitoring at home for severely ill patients

Region Stockholm develops hospital healthcare at home for acutely and chronically ill patients who so desire, with tailor-made monitoring based on the individual’s unique needs.

Sensors measure values that help caregivers work proactively, to avoid emergencies and interven­tions. At the same time, this telemedicine initiative strengthens the network around patients through increased collaboration between hospitals and home healthcare.

A regional effort to coordinate care

The project, Home Hospital Healthcare, explores opportunities and challenges for regional coordination in distance monitoring of patients with severe chronic disease, such as heart failure, arrhythmia, COPD, and endocarditis.

Patients who so wish should be able to stay at home to a greater extent without compromising patient safety. Deterioration should be detected earlier, and the risk of acute relapses reduced. A total of 8 pilot projects are planned to explore opportunities in this area.

Three companies have been selected as industrial partners through competitive tendering in a so-called Request for Information, RFI.

The project aims for a unified approach in Region Stockholm to distance monitoring with a focus on specialized and highly specialized care.
